Eligibility and registration for Dial-A-Ride and Para-Taxi services

Wheels Dial-A-Ride Service and the Para-Taxi Program is available for passengers eligible for paratransit services

If you’re unable to independently use local fixed-route service, you may be eligible for paratransit services.

Passengers eligible for paratransit services under the Americans with Disabilities Act can benefit from the Wheels Dial-A-Ride service. The criteria for ADA paratransit eligibility is the inability to independently use the fixed-route transit due to a disability or a health-related condition. A person can be found to be ADA paratransit eligible for all or for some of their transit trips, based on the individual’s specific condition(s).

1. Get the application form

Download and print or call 925-455-7500 to receive an application form by mail. 

2. Complete the application form

  • Carefully read and fill out the entire form. Incomplete applications will be returned to the sender.
  • We may contact you and/or your medical care professional for additional information. 
  • You may be asked to come to the Wheels administration offices for an in-person interview.

3. Return the application form to LAVTA

Mail the application form to the following address:

4. Process time and next steps

  • Applications will be processed within 21 days after it has been received by LAVTA.  
  • Once you have been determined eligible for ADA paratransit services by Dial-A-Ride, you will also be eligible to use other ADA paratransit services throughout the Bay Area.

You have the right to appeal if you have been denied paratransit eligibility.

If you disagree with the conditions of your ADA paratransit eligibility, or if you have been denied paratransit eligibility, you have the right to appeal the decision.  

Submit a written appeal within 60 days of your determination letter
To do so, you must submit a written appeal within sixty (60) days of your determination letter to LAVTA (Attn: Dial-A-Ride Appeals, Livermore Amador Valley Transit Authority, 1362 Rutan Dr., Suite 100, Livermore CA, 94551) and indicate whether you would like to request an appeal hearing or include your reasons for appealing in writing. Your request for an appeal will be forwarded to LAVTA’s Executive Director.

An appeal hearing will be scheduled within 30 days
Once we receive the letter, if requested, an appeal hearing will be scheduled within 30 days. The appeal hearing process will include a meeting between you (or someone on your behalf) and the appeals board, and the possibility of a functional and/or cognitive testing procedure. A response by the Executive Director will be completed within thirty days (30) of the hearing or receipt of the written appeal. Should any decision take longer than 30 days, paratransit service will be provided until a decision to deny the appeal is issued.

You may bring anyone you wish along to the meeting to speak on your behalf. Dial-A-Ride will provide free transportation for you to and from the appeals meeting.
